WHY:
Healthy people who get the flu can pass the virus to others before they know they have it. Get your flu shot on Wednesday, 9/30/20, at 11:30am in the Remick Acres Community Room.
HOW:
Call 323-8511, to RESERVE your shot. We have a limited supply of flu serum. Do not wait - CALL NOW.
**Then meet the nurse at the Community Room on 9/30.
**If you are unable to get to the Community Room, the nurse can go to your apartment. Call prior to Wednesday, Sept. 30th, to make arrangements.
PAPERWORK:
A consent form will be provided for you to complete and sign at the Clinic and given to the nurse.
DONATION:
Recommended Donation is $20. If donation is a hardship it will be waived.
REQUIRED:
Mask must be worn (over mouth and nose)
Maintain 6ft social distancing
